Click on ADD in the right horizontal toolbar in Patient List window, to add a new patient.

Enter Pt/Scrn.ID (this is the number by which the patient is identified in the system) and patient overview data.

You can enter date of Informed Consent and/or the visits and procedures performed if you want to track this components.

You can deliver the drug kits if applicable.

You can enter the problem and AE if applicable.

Track the SDV list if applicable.
Track the CRF and DCF if applicable.

Change Status to 'Enrollment Failure' and select appropriate Not Entered Reason.
